Referrals
The Outreach Support and Advisory Service works in close collaboration with a wide range of health and educational professionals to support children and young people who have physical difficulties in mainstream schools. Although the children supported are of statutory school age, we welcome referrals from children of pre-school age to enable us to contribute towards a smooth transition into school.
Please note that we now receive all referrals from the SMAP panel.
Referrals can be made by writing to senadmin@plymouth.gov.uk either as part of an EHCP application process or in form of a letter requesting support. You may be directed to our service as a direct outcome of the SMAP process.
Once we have been directed to work with a child via this process, we will make contact with you to complete the referral forms for both school and families to ensure we are adhering to data protection principles.
When you use our referral form please provide as much information as possible. Any additional information such as IEP's, annual reviews and medical reports are welcomed.
What happens next?
Within four weeks of receiving new referral information (during term time), we will make contact with the SENCo/Teacher/Key Worker to arrange an initial visit to the school/setting. On the initial visit we will observe and assess the child's needs and offer support and advice to enable them to enjoy a successful and inclusive experience. To enable this visit, forms must be signed and returned to Woodlands School.
It is essential that we receive a signed parental permission form for this initial visit to take place.
